Background: Quality improvement in delivering healthcare services is the main mission of every health system. Safe surgery checklist was introduced by World Health Organization (WHO), and hospitals were required to use it to reduce the risk of surgery-related mortality and morbidity. This study aimed to determine study hospitals’ compliance with safe surgery checklist in 2016.
Methods: A retrospective, descriptive study was done in training general hospitals affiliated by Qom University of Medical Sciences from April to June 2016. All surgery checklists in this period were collected, and its use regarding to each operation was mentioned. Data were entered into SPSS version 20 and analyzed through descriptive statistical tests and One-way ANOVA.
Results: A total of 1771 surgeries in six university hospitals performed in three months study period and their related surgery checklists were investigated. The overall use and completion rate of checklists were 74%. Among which the sign-in, time-out, and sign-out items compliance were 58%, 16%, and 26% respectively.
Conclusion: The overall compliance rate of safe surgery checklist was satisfactory except for time-out and sign-out processes. Complimentary training and regular assessment of checklist utilization could be suggested to ensure the positive attitude among operating room’s staff about the value of such a safety tool in improving patient safety.

Background: During 2009, Indonesia reported human cases of pandemic influenza A(H1N1) virus. Although the fatality rate was not as high as for H5N1 virus infection, genetic information has not yet been published. We generated whole-genome sequences from 28 influenza A(H1N1)pdm09 isolates during the first wave of the 2009 pandemic in Indonesia to determine the genetic characteristics related to clades, receptor-binding sites, antiviral resistance, virulence, pathogenicity, and polymerase activity.
Method: We detected, amplified, and sequenced the influenza A(H1N1)pdm09 viruses using real-time PCR, egg culture, reverse transcriptase PCR (RT-PCR), and Sanger sequencing. In addition, we analyzed the genetic characteristics using BioEdit and MEGA6 software.
Results: The phylogenetic analysis of the hemagglutinin (HA) gene showed similar clusters to one of the first 2009 pandemic viruses, A/California/07/2009. Resistance to amantadine in the matrix (M) gene was suggested by a fixed mutation, S31N. No mutations were found in the polymerase acid (PA) gene (T515A) or polymerase basic 2 (PB2) (E627, D701). An analysis of the neuraminidase (NA) gene did not indicate the presence of a deletion mutation, and the NS genes of the viruses did not contain the ESEV motif of the PDZ-binding domain.
Conclusion: This study produced comprehensive information through the genetic analysis of influenza A(H1N1) pdm09 viruses isolated during the 2009 pandemic outbreak. The findings of this study have increased the level of awareness of the importance of surveillance for monitoring the evolution of influenza viruses in Indonesia.

Background: growth retardation and nutritional deficiencies is common in children with cerebral palsy and can influence their motor function.
Objectives: This is a cross-sectional study aiming at evaluation of growth and minerals status in children with CP , finding association between macro and micro minerals deficiencies and some factors like age, sex, type and severity of CP and severity of malnutrition in these children.
Patients and methods: One hundred patients with CP underwent anthropometric measurements and Malnutrition was assessed according to Gomes, Waterlow, Cole, and WHO classifications.Ten patients were excluded from study due to hemolysis of blood samples. Serum analysis for mineral’s concentration was done. Also motor functions were evaluated in these children according to GMFCS (gross motor function classification system) and MACS ( manual ability classification system) classifications and statistical analysis of patient’s data was done using Chi-square and ANOVA tests.
Results: Of these 90 participants 30% had severe wasting and 38.9% had severe stunting. What is new in our study is we found zinc deficiency is common among these children in comparison to previous studies. Also We found an association between serum P level and age variant (p=0.03) and between high grades of GMFCS and Zn deficiency (p=0.04). Also correlations were significant between severe malnutrition and spastic quadriplegic type and between stunting and wasting with spastic quadriplegic CP.
Conclusion: Malnutrition and growth retardation is common in children with CP. As improvement in nutritional status has an impact on motor function and life quality in children with CP, more attention is recommended to be paid on their nutritional status and feeding problems.

Background: Phylantus niruri linn (PNL) extract has potential cytotoxic effect on colorectal cancer both on in vitro and in vivo study, but have never been reported in human subject. The purpose of this study is to elaborate whether PNL extract also has cytotoxic effect on human colorectal cancer.
Method: A one group pre-test and post test design was conducted on 15 patients with non obstructive operable rectosigmoid cancer without neo adjuvant chemotherapy or chemo radiation. Before PNL extract (100 mg Stimuno) treatment, all patient underwent rectoscopy biopsy. Stimono treatment was given at least for 14 days and then patients underwent rectal resection. Speciment from biopsy and rectosigmoid resection were manage by Department of Anatomical Pathology Faculty of Medicine Gajah Mada University Yogyakarta for evaluating the diagnosis, infiltrating lymphocyte, apoptosis index, perforin, granzyme-B and caspase-3 expression. A pair t test was done to evaluate the significant increasing measured variables and linear regression was done to evaluate the role of individual variable.
Result: Infiltrating lymphocyte, expression of perforin, granzyme-B and caspase-3 and apoptosis index were significantly (p=0.000) improve after PNL extract treatment. After linear regression, variable which significantly correlate with apoptosis was only infiltrating lymphocyte.
Conclusion: PNL extract increase apoptosis index of colorectal cancer through increasing of infiltrating lymphocyte.

Background: In this connection, we put in our work the following aim: to define the potential of simvastatin and non-medication combined impact on regular blood elements' aggregative features of patients with arterial hypertension and dyslipidemia.
Methods: In our investigation, we took 55 patients of middle age with arterial hypertension of 1-2 degree, risk 3 with dyslipidemia of IIb type. For dyslipidemia correction, all the patients were prescribed simvastatin, hypolipidemic diet and graduated exercise. Registration of clinical and laboratory indices was made before the start of treatment, in 6, 12, 18, 52 and 104 weeks of therapy. We applied biochemical, hematological and statistical methods of investigation. Control group was composed of 26 healthy volunteers of the middle age examined once.
Results: Application to patients of simvastatin, hypolipidemic diet and graduated exercise during 104 weeks positively influenced lipid composition and level of lipid peroxidation in plasma and regular blood elements taking the given indices on the control level for 12 weeks. Fulfilled therapy normalized in patients with arterial hypertension and dyslipidemia erythrocytes' aggregative abilities (aggregates' quantity decreased by 47,1% during 6 weeks, and ability to platelets' aggregation (aggregation inhibited with ADP on 74,1%, with collagen - on 47,1%) and neutrophils (aggregation weakened with lectin on 58,3%, with phytohemagglutinin - on 37,2% during 12 weeks.
Conclusion: Simvastatin reception on the background of non-medication by patients with arterial hypertension and dyslipidemia normalizes lipid composition and level of lipid peroxidation in plasma and regular blood elements for 12 weeks of investigation. These patients' therapy lowers erythrocytes' aggregative abilities for 6 weeks, and platelets' and neutrophils' - for 12 weeks of therapy.

Background: Cooperative learning considered as one of the greatest innovations in the educational system. Jigsaw learning technique, one form of cooperative learning method, has been successfully used to improve academic achievement among students. The present study was conducted to compare the effectiveness of jigsaw technique with lecture-based teaching method for medical students’ physics course. Methods: A pretest was performed on the students to ensure their literacy. Relevant topics, i.e. bioelectricity (first subject) and application of electricity for diagnosis and treatment (second subject) were then presented to the two groups using mentioned methods. A questionnaire with a confirmed validity and reliability and a posttest were used to measure their learning and assess whether it was satisfactory or not. Results: The mean posttest scores differed significantly between the two groups (p < 0.001 for subject 1 and subject 2). More than 80% of the participants agreed or agreed with all the items that assessed their satisfaction with what they had learned in Jigsaw class. However, some of them were concerned about the sufficiency of their learning and their final score. Conclusions: Jigsaw learning technique is recommended as a proper method for teaching physics if the students’ concern about their final score can be resolved and the accuracy of the information conveyed to them is ensured.

Background: Numerous factors may underlie an acute transfusion reaction. The readily available data showing the prevalence and the risk factors of transfusion reactions in Bali are scarce. Objectives: Our study aimed to reveal the prevalence and the risk factors of acute blood transfusion in Pediatric Patients in Sanglah General Hospital, Bali, Indonesia. Methods: A cross-sectional study using consecutive sampling was conducted from August 2015 to August 2016 (13 months). We collected the data from the medical records of our pediatric patients in the pediatric wards, neonatal intensive care unit (NICU), and pediatric intensive care unit (PICU). We examined the relationship between the occurrence of acute transfusion reactions and the types of blood transfusion, the history of blood transfusion, and the age of the patients. Results: A total of 107 acute transfusion reactions occurred from 3,251 blood transfusions. When acute transfusion reactions occurred, the patients were more likely to be over 12 months old compared to 0 to 12 months old (POR=2.81, 95%CI 1.78-4.58, p<0.05). Alternatively, when the transfusion reactions occurred, the patients were more likely to receive thrombocyte concentrate transfusion compared to other blood components (OR=8.11, 95%CI 5.36-12.31, p<0.05). Conclusion: The prevalence of acute transfusion reactions was 3.3%. The pediatric patients who had acute transfusion reactions were more likely to be over 12 months old or more likely to receive a platelet concentrate.

Background: One of the indices for measuring the performance of any organization including hospitals is the satisfaction of their customers. The purpose of satisfying is client judging of the rate of his expectations from services that they had taken. Patients are expected to care taken by individuals that having a mutual understanding. In the major studies role of traits behavioral and intelligence variety of the medical staff is highly significant on the patient satisfaction, including the intelligence is spiritual intelligence that could help the relationship between staffs and patients. Having high intellectual intelligence help to medical staffs to understand their services meaningful and cope better with the pressures of work. Despite numerous studies which did not consider the effect of a variety of the medical staffs’ intelligence on the patients' satisfaction. In line with this lack, the purpose of this paper is to investigate the relationship between the spiritual intelligence of nurses with patients' satisfaction with nursing care. Methods: This was a descriptive-analytical study that evaluates nursing staff and patients admitted to hospitals in Kermanshah. To collect data, spiritual intelligence and patients' satisfaction questionnaire were used. The data were analyzed by software SPSS22, using descriptive statistics (percent, diagram, mean) and Pearson correlation coefficient. Results: the results showed a significant relationship between nurses spiritual intelligence and patient satisfaction(r=.69, p<0.05). 30% of the nurses had average spiritual intelligence, and patients' satisfaction was desirable. The most satisfying was from the area of utility services (66.9%), the satisfaction of nurses (98%), and satisfaction of doctors (93%). The lowest satisfaction is in the field of patient education and receives information (31.53%). Conclusion: There was a meaningful relationship between nurses’ spiritual intelligence and patient satisfaction. Therefore, considering spiritual dimensions in staffs can increase the quality of care and patient satisfaction.

Background: Preeclampsia is the leading cause of morbidity and mortality in pregnant women and the fetus. In preeclampsia, glomerular endotheliosis occurs due to endothelium dysfunction in the kidney. Supplementation with L-Arginine is postulated to improve glomerular endotheliosis. The aim of this study is to investigate the effect of L-Arginine on improving glomerular endotheliosis in an animal model of preeclampsia.
Method: This is an analytical experiment research which was conducted in November 2016 to January 2017 in Biomedical Laboratory Faculty of Veterinary Medicine, University of Airlangga, Surabaya. Thirty pregnant mice (Mus musculus) were used in this research. The mice were divided equally into three groups, normal pregnant mice (K1), pregnant mice model of preeclampsia (K2), and preeclampsia pregnant mice treated with L-Arginine (K3). The kidneys were then obtained surgically and examined with immunohistochemistry examination. Data were analyzed by using one-way ANOVA and Post-hoc test.
Results: The mean score of glomerular endotheliosis were 0.44±0.42 for K1, 1.34±0 for K2, and 0.62±0.45 for K3. There was a significant difference between K1 and K2 (p=0.0000), and also between K2 and K3 (p=0,000).
Conclusion: L-Arginine is effective on improving glomerular entoheliosis in a mouse model of preeclampsia.

Background: Pediatrics thyroid cancers tend to be at more advanced stages at the time of diagnosis and have higher frequency of recurrences than adulthood thyroid cancers. The aim of study was to evaluate clinical characteristics and outcome between children and young adults patients with Differentiated Thyroid Cancer (DTC) treated in our hospital. Material and Methods: The medical records of 144 patient with DTC who underwent thyroid surgery followed by radioiodine and thyroid hormone suppression were retrospectively reviewed. Subjects consisted of 43 patients who were younger than 21 years old, and 101 young adult patients (older than 21 years old but younger or equal to 40 years). The clinical characteristics and outcomes were analyzed and compared, then Recurrence Free Survival (RFS) was evaluated using Kaplan-Meier Methods. Results: Female has higher tendency to have thyroid cancer than male (p = 0.006). Based on histopathology report, classic papillary thyroid cancer is the most common cancer type in children than young adult. However, there was no significant difference between two groups regarding thyroid cancer size and multifocality (p=0.815 and p=0.370). The risk of recurrent ratio of children to young adults is 3.88 (95% CI 1.38; 10.91). Similar result trend is shown for sex type, histopathology type, number of nodules, surgical technique and metastasis parameters. (adjusted HR = 7.91, 95% CI 2.11; 29.67). Conclusions: Differentiated Thyroid Cancers in children show more aggressive behavior compared to young adult patients.

Background: The prevalence ratio of heart failure is significantly increased, especially in low and middle-income countries. Adoption of self-care behaviors effectively and efficiently might be reducing the costs in result of heart failure outcomes and complications from the disease. This study aimed to determine the effects of the continuous care model on self-care behaviors in heart failure patients.
Methods: A Randomized controlled trial with Pre-test/Post-test control group design was conducted on some 80 patients with heart failure. The subjects in this study were randomly assigned to intervention (n=40) and control group (n=40). In the experimental group, Continuous care model applied for a period of three months from April to the end of June 2016, and self-care behaviors in heart failure patients in the two groups before and after the implementation of continuous care models were compared. Data collection tools included two parts: Demographic characteristics and specific questionnaire of self-care heart failure index. The data were analyzed by SPSS v. 20, chi-square, independent T, and paired test.
Results: The mean score of the Self-Care Behaviors before the intervention, at the intervention and control group were 28.33 ± 12.72 and 26.99 ± 1.96 respectively (P= 0. 663). But after the intervention, mean score of the Self-Care Behaviors in the intervention and control groups were 50.86 ± 11.96 and 28.93 ± 10.96 respectively (P < 0.0001).
Conclusion: Implementation of the continuous care model can lead to improve self-care behaviors in heart failure patients.

Background: Sunscreens are cosmetic products which are used to protect the skin from ultraviolet (UV) rays exposure. Ultraviolet rays, especially ultraviolet B (UVB) rays can cause skin cancer. C-phenylcalix-[4]-resorcinaryl octacinnamate (compound 1) and C-methylcalix-[4]-resorcinaryl octabenzoate (compound 2) are newly synthesized compounds which have high absorbance of UVB rays. Despite their potential activity, these compounds have low solubility in water. Therefore, in order to increase the solubility, they were formulated into a nanoemulsion.
Objective: The aim of the present study was to prepare and characterize a self-nano emulsifying system of two compounds as UV absorber.
Methods: Optimum formula of nanoemulsion was analyzed using D-optimal mixture design with Design Expert® ver. 7.1.5 by varying concentrations of candlenut oil, tween 80, and polyethylen glycol 400 (v/v). Sun Protection Factor (SPF), transmittance and time of emulsification were evaluated as optimization parameters. The optimal formula was further evaluated for its distribution, droplet size, and zeta potential.
Results: A combination of two compounds was used in formulations and has SPF value of 52.189. The composition of optimal formula was obtained at 2.5 mL of candlenut oil, 5.066 mL of tween 80, and 2.434 mL of polyethylene glycol 400. The results showed that the optimal formula has SPF value of 81.577, % of transmittance 90.84, emulsification time of 17.42 seconds, desirability of 0.726, average droplet size of 129.9 nm with polydispersity index 0.393 and zeta potential of -15.2 mV.
Conclusion: Nanoemulsion formulation using candlenut oil could increase SPF value of sunscreen compounds and potentially be used as alternative UV absorbers dosage forms in the futureFull Article (HTML) | download PDF

Introduction: The prevalence of stroke is increasing among young populations in Iran and among adult populations in other countries. The highest risk factors are related to cardiovascular diseases and blood pressure, and the most prevalent type of stroke is hemorrhagic stroke. The present study was aimed at comparing individual and familial situation and history of diseases in female and male patients suffering from stroke admitted in Alzahra, Feyz, and Kashani Hospitals, Isfahan in 2015-16.
Method: The present study is a descriptive quantitative research. The statistical population included all patients with stroke who had referred to Alzahra, Feyz, and Kashani Hospitals in Isfahan. The study sample consisted of 180 records documented in those hospitals. The study instrument was a researcher-designed checklist. The collected data were analyzed by descriptive and analytical methods using SPSS software.
Results: The highest number of mortality caused by stroke was related to hemorrhagic stroke to which hypertension is known as the risk factor. It was also concluded that there was no significant difference between female and male patients in terms of age, sex, place of residence, and marital status.
Conclusion: According to the results of the present study, all patients suffering from stroke experience various physical problems; therefore, different factors cause stroke, and physical disabilities are predictable in such patients, and new protocols are required to improve life conditions among such patients.

Introduction: Due to ever-increasing changes and complexities of social relationships in the present era, it is essential to prepare individuals especially young generation to deal with difficult situations. Training life skills is a must because it can promote psycho-social abilities. Objective: This research aimed to study the effectiveness of training life skills on the mental health of female students of 2nd-grade high school residing in Bam, Iran. Method: This is a quasi-experimental study with two groups with pre-test and post-test design. General Health Questionnaire (GHQ-28) was employed to collect the data. A sample of 40 students was selected using convenience sampling. They were selected out of students who visited Payam-e Hedayat Educational-Cultural Complex to participate in GPA Promotion Program. They were randomly assigned to control and experiment groups. 16 sessions of training were held. After one month, post-test was performed. The data were analyzed using MANOVA. Result: Findings showed that training life skills caused a significant difference in mental health compared to pre-test. Conclusion: It is therefore concluded that training life skills can increase the mental health of female students on the 2nd-grade high school of Bam, Iran.

Background: The new anticancer is urgently needed due to the high resistance and recurrence of breast cancer. Previous study reported that a new chalcone derivative, (e)-1-(4’-aminophenyl)-3-phenylprop-2-en-1-on, has a potential cytotoxicity against T47D breast cancer cell line. In this study we investigated the anticancer activity of (e)-1-(4’-aminophenyl)-3-phenylprop-2-en-1-on against DMBA-induced mammary cancer in Sprague Dawley rat and its effect on microRNA-21 expression.
Methods: Twenty four female rats were divided into 6 groups. The first group, G1 received corn oil. The groups 2 to 6 (G2, G3, T1, T2, and T3) were induced by DMBA (dissolved in corn oil) 20 mg/kgBW for 5 weeks. After breast nodule observed, G2 received vehicle and G3 received tamoxifen. Whereas, T1, T2, T3 received (e)-1-(4’-aminophenyl)-3-phenylprop-2-en-1-on with different doses, ie. 5, 15, and 45 mg/kgBW/day for 21 days, respectively. Tumor size was measured every weeks for 21 days and on day 22, plasma and breast tissues were collected to examine the miR-21 expression by qRT-PCR and histopathological feature, respectively.
Result: The result showed that significantly decreased tumor growth (p<0.05) and better histopathological malignant grading in G3, T1, T2, T3 were observed. Moreover, significantly reduced miR-21 relative expression (p<0.05) in G3, T1 and T2 were also observed.
Conclusion: (e)-1-(4’-aminophenyl)-3-phenylprop-2-en-1-on has potential anticancer activity on DMBA-induced mammary cancer in Sprague Dawley rat through its activity on miR-21 expression. Hence, (e)-1-(4’-aminophenyl)-3-phenylprop-2-en-1-on might be a new anticancer candidate in the future.

Introduction: People with diabetes have delayed sleep compared to ordinary people and do not have good sleep quality because of impaired glucose level. Saffron effect on insomnia in several studies on animals have checked, and beneficial results for it have observed.
Methods: This study was a quasi-experimental study aiming to determine the effect of oral saffron capsules on sleep quality in diabetic patients in the city of Zabol in 2016. This study conducted on 50 diabetic patients in Zabol city which were qualified for inclusion criteria for the study. The sleep quality of patients assessed with Pittsburgh Sleep Quality Index (PSQI). Then the intervention was done as a daily intake of one 300 mg capsule of saffron in the intervention group and a placebo capsule in the control group, for a week, every day between hours 12 to 14 am and after taking lunch. After a week, sleep quality measured again.
Results: After the intervention, the comparison of the mean scores of two groups showed a significant difference (P = 0.001). Also, comparison of a total mean score of sleep quality in patients before and after intervention showed statistically significant difference in two groups (p = 0.001).
Conclusion: Results of this study showed that daily administration of saffron oral capsules improves sleep quality in diabetic patients. Saffron can use for improving sleep quality in diabetic patients along with other existing therapies.

Background: Acute Myocardial Infarction (AMI) is an emergency medical condition which still has high mortality and morbidity. In recent decades, its prevalence has a tendency to increase, parallel with other chronic diseases. One of suspected contributing factor in AMI is the presence of C1562T polymorphism on MMP-9 gene which increases plasma MMP-9 concentration and destabilizes the plaque. However, this notion needs further confirmatory studies, as there are several contradictive reports regarding their association. This Study aimed to determine the relationship between C1562T polymorphism with the increase of MMP-9 concentration in AMI. Method: A cross-sectional was conducted in Cardiovascular Centre of Sanglah General Hospital. The peripheral blood samples were obtained from subjects, and the DNA and blood plasma was isolated. The polymorphism was detected using PCR and RFLP while MMP-9 concentration was determined by ELISA. Results: The average concentration of plasma MMP-9 was found to be at 8.33 ng/mL which ranged from 0.74 ng/mL until 31.93ng/mL. The proportion of MMP-9 C1562T polymorphism was 20% of all subjects. The average concentration of MMP-9 was higher than cut-off standard which is 0.6 ng/mL. Analysis within the group revealed that CT-genotype had significantly higher average MMP-9 compared with CC phenotype (11.27 ng/mL vs. 7.65 ng/mL). Conclusion: C1562T polymorphism appears to be significantly associated with AMI by increasing the concentration of MMP-9 in blood plasma.

Background: Checking the health status is one of the main components of nursing care. Failing to use active and learner-based educational methods might cause failures in analysis skills and clinical examinations.
Objectives: This study was conducted in order to study the effect of blended training (programed and lecture-based training) on learning health status assessment course among nursing students.
Design/Settings/Participants/Methods: Over a semi-experimental research with two post-test groups, 60 freshmen nursing students in two educational centers were selected through convenient sampling method and randomly placed in intervention (30 people) and control (30 people) groups. As for the intervention group, the instructor held four two-hour of health status course every week in blended method (programed and lecture-based education). The same content with the same number of sessions was delivered by the same instructor just through lecturing method. The tool used to collect data was a researcher-made test composed of two sections of demographic information and 40 multiple-choice questions. SPSS 15 and statistical tests such as chi-square and independent t were used to analyze the data.
Findings: the average post-test scores of health status were obtained for both blended (11.50 ± 1.45) and lecture-based (10.55 ± 2.01) educational methods. Independent t test for groups indicated a statistically significant difference between the post-test results of the two groups (P = 0.04).
Conclusion: blending lecture-based educational method and individual education methods such as programed education can enhance the learning quality of students.

Background: The Child-Pugh score is an internationally accepted system for grading the severity of chronic liver disease such as cirrhosis. It assesses the severity of the liver disease to avoid subjectivity and inconsistency. However, some non-invasive serological markers may also be useful to evaluate the stage of liver fibrosis in determining the severity of cirrhosis. Some of the known markers are Arginase-1 (ARG-1), IL-13, fibronectin (FN), tissue inhibitor of metalloproteinases-1 (TIMP-1), and metalloproteinases-1 (MMP-1). Objective: To evaluate the correlation between the circulating levels of ARG-1, IL-13, FN, TIMP-1, MMP-1 with the score Child-Pugh in cirrhosis patients. Materials and Methods: We conducted a cross-sectional study enrolling fifty-six patients of the chronic liver with Child-Pugh A, B, and C. The score was calculated based on the value of serum bilirubin, serum albumin, internationalized normal ratio (INR), and the severity of ascites and hepatic encephalopathy. Serum ARG-1, IL-13, FN, TIMP-1, MMP-1 were analyzed using an enzyme-linked immune sorbent assay. The differences were analyzed using Mann-Whitney test, Spearman’s correlation (univariate analysis), and multivariate analysis by the multi-regression test. Results: There was a significant correlation of ARG-1 serum with Child-Pugh scores (r = -0.589, p<0.05). However, we did not find a significant correlation between the Child-Pugh scores and the IL-13 (r=0.238, p=0.078), FN (r=-0.151, p=0.265), TIMP-1 (r=-0.158, p=0.244), and MMP-1 (r = -0.006, p = 0.967) with the Child-Pugh scores. Conclusion: The serum marker of ARG-1 was found correlated with the severity of the chronic liver disease.

Objective: Antipsychotic drugs are the first-line therapy of psychotic disorders. The aim of this study is to compare the response rapidity of the first and second generation antipsychotics in the treatment of acute psychosis. Methods: In a randomized, double-blind, controlled clinical trial, thirty patients with acute psychosis were randomly allocated into three groups and treated with each of the three antipsychotics: Aripiprazole, risperidone, and perphenazine. The onset of response to each drug was assessed by the Positive and Negative Symptoms Scale. Results: Initial response was seen in 66.6 % of subjects during two weeks of intervention. The mean time of response in the risperidone group was 9.6 days, in the aripiprazole group 11.1 days, and in the perphenazine group 11.3 days. Individuals who received risperidone reached the event sooner than the other two groups. Conclusion: The result of this trial suggested that the response rate of the proposed three drugs was equal, but the onset of action of aripiprazole was slower than risperidone and perphenazine.

Introduction: Thalassemia is the most common genetic disorder in the world. Children with major thalassemia face much lifelong stress and anxiety related to invasive methods of treatment including venipuncture and blood transfusion that cause discomfort and anxiety symptoms in children and changes their physiological indicators.
Methods: The aim of this study was to evaluate the effect of foot reflexology massage on vital signs and anxiety after blood transfusions in children with thalassemia. It was a quasi-experimental study, which conducted on 60 children with thalassemia who had inclusion criteria. Patients randomized into three groups of reflexology massage, regular massage, and control group. The level of anxiety in patients assessed by an anxiety measurement based on the Observational Scale of Behavioral Distress-Revised (OSBD-R) and patients’ vital signs measured 20 minutes before blood transfusion.
Results: A significant difference (p=0.0001) observed between the vital signs of children in reflexology massage group before and after the intervention. The average score for patient's anxiety before and after the intervention for reflexology massage group (p=0.003) and regular massage group (p=0.005) was significantly different, but in the control group showed no significant difference before and after the intervention (p=0.09). After the intervention, the mean score of anxiety in patients of the three groups was statistically significant (p=0.0001).
Conclusion: The results of this study confirm that foot reflexology massage affects vital signs of children with thalassemia undergoing a blood transfusion and reduces their anxiety.

Objective: To determine the correlation between smoking and Noise Induced Hearing Loss (NIHL)
Method: Analytic with a cross-sectional study
Result: Of the 122 workers, the proportion of NIHLwas 89.3%, with mild deafness (68.8%) as the most common proportion. We found that there was a significant correlation between types of smokers and severity of hearing loss (p = 0.000). Workers who smoke are likely to experience NIHL1.224 times more likely than non-smokers (PR = 1.224, p = 0.002, CI 95% = 1,077 - 1,392).
Conclusion: There is significant correlation between smoking and noise induced hearing loss

Background: Olfactory dysfunction is one of the chronic rhinosinusitis symptoms. A large number of patients with chronic rhinosinusitis without polyps whose treated by Functional Endoscopic Sinus Surgery (FESS) need to be investigated using Sniffin ' sticks test before and after surgery. Objective: The aim of this study is to investigate the role of olfactory function improvement after FESS in patients with chronic rhinosinusitis without polyps. Methods: This study is an experimental study using one group pre-post test design. The measurement of the threshold, discrimination, identification, and TDI scores were assessed by Sniffin’ stick test in 3 periods (before FESS, 2 weeks and 4 weeks after FESS intervention). Results: The mean value of the TDI score before FESS performed was 19.43 ± 7.32 (hyposmia), 2 weeks after the operation was 26.9 ± 5.78 (hyposmia), and 4 weeks after the operation was 33.25 ± 3.86 (normosmia).There is a significant mean difference in thresholds, discrimination, identification, and olfactory function (TDI) scores in patients with chronic rhinosinusitis without polyps before FESS, 2 weeks after the procedure, and 4 weeks after FESS. Conclusion:There was a significant improvement in olfactory function after FESS performed in patients with chronic rhinosinusitis without polyps.

In humans, early-life microbiota disruption has been proven to be associated with increased risk of overweight later in childhood. As the use of cefixime is rising in pediatric clinical infections, it is important to determine the changes in gut microbiota composition after early-life exposure of this antibiotic. In this study, six-week old male Wistar rats were used as an animal model, divided equally into a control group and cefixime group. Both groups were transplanted orally with feces from 7-months old infant boy prior to antibiotic administration. Cefixime group was given cefixime orally twice a day for five days in 3 consecutive periods with one-week interval. Microbiome analysis from caecal content was conducted to determine the changes in gut microbiota composition. This study found an increase in relative abundance of Bacteroides and decrease in relative abundance of Firmicutes. An increase in Bacteroidetes/Firmicutes ratio by 32% was also observed. It was concluded that early-life-repeated-administration of cefixime in rat has a significant increase in Bacteroidetes/Firmicutes ratio.

Background: An overtraining or a practice of an excessive intensity without a sufficient recovery period may induce myocardial injury. Objective: The aim of this study was to reveal myocardial changes in overtrained rats through a histo-pathobiological analysis. Methods: We enrolled 24 male Wistar rats for 6 weeks. As many as 8 rats or the control group swam 15 minute/day, 5 days/week. The overtrained group consisted of 24 rats swam for 90 minutes, twice a day, daily. After sacrificing, the hearts were excised for pathological preparation slides. Results: The overtrained group’s slides showed an increase in chromatin fragmentation in the myocardial structure. The left ventricle wall thickness was also greater in overtrained groups than the control. Necrotic bodies were found in the myocardium of overtrained groups, but none in the control group. Conclusion: We found the negative effects of an overtraining to the myocardium. Avoiding overtraining is necessary to avoid a myocardial injury.

Background: The rapid detection of influenza viruses from clinical samples is important for establishing public health and clinical decisions, including infection prevention and control measures. The SD Bioline rapid influenza test (Standard Diagnostics, Inc., Suwon, Korea) is a point-of-care (POC) diagnostic test which based on influenza-specific monoclonal antibodies. The purpose of this study is to assess the performance of this assay against the reference diagnostic standards of RT-PCR for Influenza diagnosis in Surveillance activity.
Methods: 4,262 nasal and throat swab specimens were collected from influenza-like illness (ILI) patients from sentinel hospital and health-care sites across 10 provinces in Indonesia. Specimens were tested with the SD Bioline rapid influenza test in sentinels and rRT-PCR test was conducted in referral laboratory in Jakarta. Data analysis was performed by comparing the results of rapid test conducted in the POC settting and the results of rRT-PCR test.
Results: Testing of throat and nasal swabs from children and adults indicated the specificity of The SD Bioline rapid influenza test to be 97% for influenza A and B, respectively, in 4262 specimens. However, the overall sensitivity of the SD Bioline rapid influenza test for these samples was low at 25%.
Conclusions: Overall, the results indicated that the SD Bioline rapid influenza test has a high specificity but low sensitivity for detection influenza from throat and nasal swabs therefore it will be a challenge of laboratory testing on site.

Introduction: Nurses are the largest group working in hospitals and play a key role in health care systems. They are also considered important members of health team. Therefore, they are very effective in society's and one's health. Objective: This article aimed to study the relationship of emotion dysregulation with psychoactive substance abuse among nurses. Method: This is a descriptive, correlational study. The targeted population consisted of all nurses working in hospitals of Kermanshah, Iran in 2016. A sample of 350 was selected using multi-stage cluster sampling. However, the number reduced to 341. The data were collected using three questionnaires: Gratz & Roemer (2004) Difficulties in Emotion Regulation Scale (DERS) and Bahremand et al. (2014) Psychoactive Substance Abuse Questionnaire. The data were analyzed in SPSS22 using Pearson correlation study and regression analysis. Result: Research findings showed a significant relationship between the components of emotional dysregulation with tend to consume psychoactive drugs there, and the relationship level (P <0/001) is significant. Conclusion: So it can be concluded that components of emotion regulation predicted the psychoactive substance abuse.

Introduction: The correct alignment of femoral components is necessary to achieve a successful total knee arthroplasty (TKA) and preventing complications such as pain, stiffness, loosening, instability; which itself, depends on making perfect cuts. The posterior distal femoral cut, is one of the most important factors. Yet, the best method of its measurement is controversial.
Methods: In this study, 20 cadavers of femoral bones were used; on which, the distal femoral rotation (DFR) was measured manually by 8 orthopedic surgeons and also was calculated on the basis of the trans-epicondylar axis (TEA) by 2 orthopedic surgeons and 2 radiologists on CT scan image. Accuracy and reliability were assessed in both groups, and the risk of severe malrotation was determined according to an outlier defined as 3 and 5 degrees.
Results: The mean DFR measured manually, was 1.8±2.1º and the one attained by CT scan images was reported 3.3±1.1º, projecting a remarkable difference between two groups. The Kappa reliability index was calculated about 0.81 in CT scan results and 0.64 in the other group. (k>0.7 was considered as valuable). With an outlier defined as 3º, from the normal distal femoral rotation based on posterior condylar axis (PCA), 3º external rotation; 12% of data were outlying in the manually of measured data, and there was no outlier data were reported in the other group. (With a statistically significant difference between two groups)
Conclusion: According to the unreliability and noticeable risk of error in measuring DFR manually based on TEA, this study suggests using CT scan in the patient’s pre-op assessment to obtain the most accurate data.

Introduction: Lack of expressing opinions by employees in organizations causes organizational silence. Researchers have identified two contributing factors for the organizational silence: Management fear of negative feedback and staff perceptions of management's complex ideas about them. Method: This article aimed to forecast organizational silence according to moral behavior styles. A total of 300 employees working in Mobile Telecommunication Company of Iran (MCI) were enrolled as the sample using simple random sampling. It was a correlational study. Data were collected using two questionnaires: Organizational Silence Questionnaire and Moral Behavior Questionnaire. Descriptive statistics (mean and standard deviation) and confirmatory factor analysis used for data analysis. For the hypothesis testing, path analysis, which is a part of SEM, was employed using LISREL. Result: The results showed that organizational silence had a significant relationship with moral behavior. Conclusion: The results help managers prepare the ground to receive feedback from employees with various moral behavior styles.

Background: For practice it is still very important to search ways of early correction of arterial hypertension displays with the help of non-medication. In this connection it seems to be important to identify the impact of regular athletic exercise on early developing of arterial hypertension, hemostatic and rheological abnormalities in persons of young age.
Methods: We investigated 99 young patients with arterial hypertension of the 1st degree. The patients were divided into two comparable groups. The first experimental group consisted of 57 persons who regularly - one hour a day, six times a week, during a year - experienced athletic exercises. The second experimental group consisted of 42 persons and was composed of patients who had no wish to train physically and take medicines. Control group was composed of 43 healthy persons of the same age. In all the examined patients we fulfilled the investigation of basic indices of hemostasis, blood viscosity, erythrocytes' aggregation and deformability. The data were processes by Student's t-criterion.
Results: Patients of both experimental groups initially were noted to have hemostasis activation and negative changes of blood rheological features. In a year of regular athletic exercise patients of the first experimental group reached normalization of arterial pressure. They also had decrease of platelets' aggregative ability (aggregation with ADP 2.13±0.16 Un) and activity weakening of hemostasis plasma link (INR 1.12±0.05, APTT 29.8±0.37 s) to control level. Avoidance of exercise in the second experimental group promoted further increase of functional activity of platelets and plasma hemostasis at simultaneous worsening of relevant blood rheological features.
Conclusion: At initial stages of arterial hypertension development in case of regular athletic exercise fulfilled by young patients it is quite possible to reach normalization of arterial pressure level, hemostasis activity and hemorheology.

Introduction: It is supposed that the serum level of Zinc relates to the cirrhosis. We conducted this study to assess the serum zinc levels in patients with liver cirrhosis.
Methods and materials: In this descriptive analytic cohort, 80 patients over 18 years old, mean age 54.23±15.05 with liver cirrhosis referred to Imam Khomeini hospital outpatient’s clinic or hospitalized in the internal ward of this hospital in Ahwaz during 2015 were recruited. The severity of the disease based on MELD and Child scoring system, encephalopathy, ascites, SBP, serum zinc levels in patients were measured, and the relationship between these variables and zinc serum level was investigated.
Results: The mean serum level was 61.36±25.07 and Zn deficiency observed in 22 patients (27. 5%). Regarding the etiology of cirrhosis, viral hepatitis (B, C) was the most common followed by cryptogenic, but the Zn serum level was not significantly different considering etiology of the cirrhosis. Moreover, the Zn serum level was not significantly different between groups with different ascites severity and Child and MELD scoring system, but the difference was significant between groups with or without SBP and encephalopathy. Using regression test we showed that the correlation of Zn serum level with spontaneous bacterial peritonitis(SBP) and encephalopathy was significant and was not significant with etiology of cirrhosis, ascites and Child and MELD scoring system.
Conclusion; we discovered that Zn serum level in cirrhotic patients was lower than normal range (70micg/dl), moreover we proved that lower concentration of Zn is correlated with encephalopathy and spontaneous bacterial peritonitis.

Introduction: Hospitalization for surgery is associated with stress and anxiety for most patients. Candidates for Coronary Artery Bypass Grafting (CABG) experience a lot of stress and tension. This study aimed to identify and explore the determinants of stress in CABG patients.
Methods: This qualitative study builds on content analysis. The sample consisted of 21 CABG patients. Data was collected via unstructured interviews, and the sample was selected based on purpose and with maximum variation. The process of data analysis proposed by Graneheim and Lundman was followed. Credibility and dependability of the study were assured according to Guba and Lincoln’s evaluative criteria.
Results: The results were categorized into three main themes and five categories in terms of nature. The three-emerging themes include "presentation", "situational determinants", and "uncertainty".
Conclusion: Mental stress affects the length of stay in the hospital, postoperative pain, and quality of life. It is necessary to identify the most important stressors perceived by CABG patients, to control stress, and hence to comfort patients mentally by healthcare providers.

Background: Iron dust is one of the many irritant substances present in the environment and acts as a pollutant in the air. Exposure to iron dust substances in the air is thought to result in disruption of nasal mucociliary transport when inhaled. PT. Gunung Gahapi Sakti Medan is one of the companies engaged in iron smelting by employing quite a lot of workers.
Objective: To know the relation of exposure to iron dust to the disruption of nasal mucociliary transport time at factory workers PT. Gunung Gahapi Sakti.
Method: The type of research conducted is an analytical study with cross sectional study design (crossectional study).
Results: Of 322 people, a total of 97 workers were selected based on inclusion and exclusion criteria. 18-28 years of age have more nasal mucociliary transport time ≤ 17 minutes, while employees aged 29-39 years and 40-50 years have a nasal mucosiliar transport time> 25 minutes. Women have a nasal mucociliary transport time ≤ 17 minutes, while male employees have mucociliary transport time of the nose> 17 minutes. The duration of 1-10 years has a mucociliary transport time of the nose ≤ 17 minutes, whereas those who have worked 11-20 years l have time Nasal mucociliary transport> 17 minutes. Workers on close exposure have a mucociliary transport time> 17 min, while exposure has a mucociliary transport time ≤ 17 min
Conclusions: The nasal mucociliary transport time of employees is prolonged or decelerated with increasing age and duration, and the mucociliary transport time of the nose in male employees is longer than in women. The workplace (location of exposure) also significantly affects the mucociliary transport of the nose.

Background: Ethics is a curriculum subject, but it is also a manner of being a professional as well. It is necessary to increase the nurses’ ability in making ethical decisions and pay attention to academic training. Materials and Methods: This study is a descriptive research that was conducted on 127 nurses and nursing students to study the level of awareness of nurses and students about professional nursing ethics and to find the causes influencing such awareness. A nursing profession ethics questionnaire was used as the research tool. The data was then analyzed using SPSS16 statistical software through descriptive statistics, Chi-square, independent t-test, and Pearson methods. Results: There was no significant relationship between professional ethics score and personal details such as age, sex, academic semester, GPA, and being a nurse or student (p>0.05). However, in the students’ group, there was a significant relationship between an interest in the field of study and the ethics score (p<0.05). There was no significant relationship between the ethic score of students’ group and nurses’ group (p>0.05). The mean score of ethics of both groups of students and nurses was at a medium level. Discussion and Conclusion: According to the results of the study, moral judgment of both students and nurses’ groups in clinical settings was evaluated as medium level. This is considered insufficient for”morally demanding” jobs like nursing. Therefore, there is a need to increase the efforts in the field of ethics education and continuous assessment of the ethical performance of the nurses.

A comparison of the effects of every day and every other day administration of Granulocyte - colony stimulating factor (G-CSF) on the number of leukocyte, platelets, hemoglobin, and pain in patients with breast cancer

Background: Prophylaxis fever and neutropenia using G-CFS are recommended in most chemotherapy regimens in women with breast cancer. Among common side effects of G-CFS injection, one can refer to skeletal pains. The present study was carried out in order to examine the level of pain and changes in blood cells in two methods of every day and every other day administration of G-CFS.
Methods: In the present clinical trial, 186 patients (93 in each group) were considered as the study sample. Ninety-three patients with breast cancer were given G-CFS on a daily basis, and then every other day in the next phase of their chemotherapy, the effect of these two methods on the number of WBC, Hb, PLT, and pain level in the patients were compared.
Results: There was no significant difference between the two administration methods regarding the number of white blood cells and platelet of the patients. The mean score of pain in every other day administration of G-CFS was 5.6 while it was 6.6 in the group with a daily administration, and this difference was significantly meaningful (p = 0.016). In patients with breast cancer, every other day compared to daily administration of G-CFS did not lead to any difference in the number of white blood cells and platelets.
Conclusion: There was no significant difference between the groups received G-CFS on an everyday basis and the one on an every-other-day basis in terms of leukocytopenia. It is recommended that G-CFS is administered on an every-other-day basis

Introduction: Depression is a chronic but curable disease that early diagnosis and treatment can lead to a higher life quality. A supplementary treatment such as exercise can reduce depressive symptoms. Tai Chi Exercise is easy to learn and easy to do for older adults in nursing homes. This study investigated the effect of Tai Chi exercise on depressive symptoms in elderly residents at Sadeghieh Nursing Home.
Method & material: This is a pre-test, post-test experimental study. Subjects included 62 elderly adults above 60 years of age who were randomly divided into four experimental subgroups and a control group. Participants were older adults who had not been participated any exercise activity during the previous month. The ten step Tai Chi exercise performed three times per week for twelve weeks in experimental groups and a control group doing only their activity of daily living(ADL). The Beck depression Inventory Second Edition(BDI-II) was used to assess depression.
Results: This study shows a statistically significant difference between depression mean scores in the experimental group compared to the control group(p<0.01). After adjustment of The mean older adults age, the differnece remained significant.
Conclusion: This study showed that Tai Chi Exercise helps reduce depression in the older adult in residential homes. Social interactions thorough Tai Chi sessions and a sense of group belongingness may help reduce depression in older adults.

Introduction: Self-care program makes it easy to cope with chronic conditions such as advanced renal diseases. So this study has done in order to evaluate effect of self care on patients undergoing hemodialysis .
Methods: This research is a Quasi experimental study conducted in the Sanandaj Hospitals affiliated to Kurdistan University of Medical Sciences in 2016. 60 patients were selected based on inclusion criteria and non-random purposive sampling in two intervention and control groups (each group 30 people). The 3-part questionnaire was used to collect data. The first part included demographic data , second part consisted of questions about training needs, and the third section consists of questions to assess the self-care of the studied samples. Data analyzed using SPSS version 16, descriptive and inferential statistics.
Results: Results showed that Patient's age ranges were between 25 to 70 years. Mean duration of hemodialysis in the intervention and control groups was 6.14 and 6.59 years respectively. in terms of training needs before the experiment, both groups are the same (P= 63.2). the results show that self-care score before intervention (P=0.618) is the same in both groups. After the intervention, there is the difference in self-care between the two groups, but this difference was not statistically significant (P=0.312).
Conclusion: Assessment of Self-care needs in patients undergoing hemodialysis needs education programs. These patients dependence brings about some compatibility issues which could be addressed rightfully leading to improvement of their self-care activities.

Evaluation of the accuracy of Bio-impedance in predicting creatinine clearance in ICU patients with acute kidney injury; Comparison with measured creatinine clearance and calculated by formulas

Introduction
It is still of top priority to find a method to measure creatinine clearance in patients with acute kidney injury (AKI) in order to regulate drug dosage and avoid treatment failure. Therefore, the present study was an attempt to compare creatinine clearance measured with 6-hour urine and creatinine clearance measured by formulas and bio-impedance method in patients with AKI hospitalized in ICU.
Materials and methods
In the present prospective cross-sectional study, 28 adult patients with an increase in serum creatinine of at least 0.3 according to AKIN criteria and admitted in ICU of Imam Hussein Hospital were selected as the study sample. For all of the patients who entered the study, creatinine clearance was measured by collecting 6-hour urine through electrical bio-impedance method or formulas of Cockcroft and Gault, MDRD, and CKD-EPI. After the required data were collected, they were recorded in pre-prepared checklists, and finally fed into a software to be statistically analyzed. At last, the obtained creatinine clearances were compared with those measured by collecting 6-hour urine so as to specify the best formula or method to determine creatinine clearance in patients with AKI.
Results
The results showed that the bias values of bio-impedance compared to 6-hour urinary clearance, MDRD, Cockcroft, and CKD-EPI were respectively 6.34, 3.31, 5.01, and 15.14. Also, error values of bio-impedance compared to 6-hour urinary clearance, MDRD, Cockcroft, and CKD-EPI were respectively 140%, 28%, 60%, and 47%. The internal consistency of the bio-impedance values by the above methods was respectively 0.782, 0.983, 0.939, and 0.893. According to the results of the present study, bio-impedance did not have an appropriate capacity to measure creatinine clearance or estimate GFR in patients with AKI.
Conclusion
According to the obtained results, it can be held that none of the formulas MDRD, Cockcroft and Gault, and CKD-EPI formed an acceptable relationship with measuring creatinine clearance by collecting 6-hour urine as a reference method. Therefore, this group of patients needs to figure out more precise and specific methods to measure creatinine clearance and glomerular filtration.Full Article (HTML) | download PDF

Introduction: Breast cancer is the most common cancer and the second leading cause of cancer death among women. Breast self-examination (BSE) is a valuable technique in the detection of this cancer. Despite the relative benefit of BSE, its use remains low. This study aimed to investigate and discover the barriers of breast self-examination.
Materials and Methods: In this review study, an online search among articles published from 2000 to 2016 was conducted through CINAHL, PubMed, Science Direct, Elsevier, SID, Iran Medex, Magiran and Google scholar databases using key words; “Breast self-examination”, “Barriers of breast self-examination”, Breast cancer” and “Examination”. The protocol of York University Guide was used to select the articles.
Results: In total, 15 articles were used from 35 articles that were initially obtained from the search, from which, 8 were Iranian articles and the rest were non-Iranian articles. The articles indicated that, barriers of BSE can be divided into the factors related to knowledge, cultural factors (shame), psychological factors (fear and anxiety), and demographic and environmental factors, which according to the articles, the share of lack of knowledge was much higher than the rest.
Conclusion: The analysis of articles showed that, the prevalence of breast self-examination was low and self-examination of breast was not done properly, which was related to the lack of knowledge. Therefore, choosing an appropriate method of education could eliminate the barriers to BSC. In this regard, mass media have an important and essential role.Full Article (HTML) | download PDF

Giant Lower Back Lipoma in Pediatric : A Case Report

Backgrounds : Lipoma is the most common soft-tissue mesenchymal neoplasm in adults. Lipoma that sized over 10 cm and weighed over 1000 grams is considered giant lipoma. Giant lipoma, especially in children are rare, so it represent a real diagnostic and therapeutic challenge. In this case report, we report a further case.
Case : A three-year-old girl with a history of progressive growing mass on the lower back since born resulting in pain. CT scan with contrast shows soft tissue mass with fat content sized 6,1 x 12,1 x 12,7 cm without contrast enhancement. Surgical excision was performed to completely remove the tumor. Histopathology examination confirmed the diagnosis of lipoma.
Conclusion : Pediatric giant lipomas are rare, when they occur an appropriate work up must be done. This should be followed by adequate open surgical excision and repeat examination over time to monitor for recurrence.
