Bambang Heriyanto, Nike Susanti, Vivi Setiawaty Online First: October 03, 2018Characterization and Identification of Poliovirus from the Environment in Indonesia 2015
Bambang Heriyanto, Nike Susanti, Vivi SetiawatyBackground: Poliomyelitis is a disease caused by the poliovirus which causes abnormalities in the nervous system, commonly affects children and can be prevented by immunization. Since March 27th, 2014, Indonesia has been declared to be free of polio and anticipate the mutation of poliovirus type 2 (vaccine-derived polioviruses / VDPV). Indonesia will change the use of policy vaccines from triOPV to biOPV by erasing the P2 type before switching from OPV to IPV. Before the policy was implemented, it is needed to have research to determine the level of immunity of children against poliovirus and type of poliovirus which circulate in the area of OPV and IPV in Indonesia.
Methods: The research study was conducted in five cities in five provinces in Indonesia in 2015. The serum was taken on 100 children between 12-59 months and feces were taken towards 150 children in the same age range. The examination of antibody titers in serum performed using the method of neutralization.
Result: Research shows that in regions that apply OPV as routine immunization, about 94.5% of children had antibodies against poliovirus type 1, 96.8% against type 2 and 93.3% of type 3. In areas which implement IPV as routine immunization, children have gained 85% immunity against poliovirus type 1, 92% against type 2 and 93% of type 3. In the environments that implement OPV, poliovirus was found in feces specimens (0.5%) or the mixed waste (58%). All the circulating virus is a virus originated from Sabin vaccine which mostly comes from type 2 (85% of mixed waste and 60% of feces).
Conclusion: From this study, it can be concluded that sabin polio virus is still circulating in the area which applies OPV as routine immunization and poliovirus type 2 is commonly found there. Immunity inflicted on children varies in each region. Children living in a regular immunization using OPV and IPV had immunity for about 85-90%.

Ferdy Iskandar, Fabianto Santoso, Angela Franzeska, Yenna Tasia, Kamajaya Mulyana Online First: October 03, 2018Mini Health Technology Assessment of Urban and Rural Health Care in Indonesia
Ferdy Iskandar, Fabianto Santoso, Angela Franzeska, Yenna Tasia, Kamajaya MulyanaBackground: Health Technology Assessment (HTA) is a program to evaluate the use of technology in healthcare. HTA proposed by World Health Organization (WHO) will affect the government’s decision-making in the development of healthcare technology. The use of technology should be assessed from its cost-effectiveness to its potential ethical problems. HTA Unit – a division who organizes HTA in government in Indonesia – has emerged in ministry of health in 2003 and focused in drugs and vaccines problems. However, HTA in Indonesia has not been formally effective until today.
Objectives: To evaluate the usage, supply, and demand of the health technology and the spread of health technology in urban and rural areas.
Methods: This study is a cross-sectional study using questionnaire to gather quantitative and qualitative data. The respondents of this study are medical doctors in Indonesia, and the selection of the respondents is done consecutively. We divide respondents’ area into urban and rural areas, and health practitioners as field executors (general practitioners or GP and specialists) and head of healthcare facilities.
Results: The total samples are 48 health practitioners and 14 head of healthcare facilities. Statistical analysis shows that the two settings (urban and rural) are not significantly different (p=0.478), while descriptive analysis shows that the inadequacy of health technology is higher in rural than that in urban areas (70% VS 52.63%). The relationship between the field of health practitioners (GP and specialist) and the adequacy of health technology is not significant. More GP than specialists complain about the inadequacy of health technology in giving the health service to the patients. About 92.8% head of health facility said that technology is needed to support the health service, while only 35.71% agreed that their health facilities still have inadequate health technology.
Conclusion: The use of health technology in Indonesia is still inadequate. The spread of health technology is not equal in urban and rural areas. The lack of health technology in rural areas, especially for GP, affect in diagnosing and giving therapy to patients. The poor spread of technology in healthcare also make gap between health research and health clinical practice.

A.A. BikbulatovaEarly diagnostics of developing abnormalities in structure and functioning of locomotor apparatus of 5-6 year-old children, including manifestations of scoliosis, is necessary for maximally early beginning of its adequate correction. Estimation of morphological and functional peculiarities of modern children with scoliosis, living in Central Russia, is of great practical interest. The aim is to estimate morphological and functional peculiarities of locomotor apparatus of living in Central Russia 5-6 year-old children, having scoliosis of I-II degree. In our study we applied methods of morphofunctional estimation of a human being with processing of received results by Student’s t-criterion. Conducted estimation of deviation of spinal column position from the vertical one in children with scoliosis allowed us to detect its increase in them on average 4.4cm. We found lowering of humeral index value on 23.3% in taken into investigation children having scoliosis. It was accompanied by significant decrease of mobility degree of the spinal column in three planes of motion performance. It was also accompanied by lowering of mobility degree of their spinal column to the left side – on 47.4%, to the right side – on 53.6%, backwards – on 39.7%. Children with scoliosis manifestations were noted to have inhibition of morphometric characteristics’ development in comparison with the indices of the control group. While defining the values of dynamometry of children with scoliosis we noticed significant lowering of muscle strength level of both hands in comparison with the control level – on 19.4% from the left side, on 26.7% from the right side. Decrease of hands’ muscle strength of children with scoliosis was accompanied by decrease of the common strength till 34.2±0.16 kg against 36.8±0.31 kg in the control group. Tolerance of body muscles of children with scoliosis was significantly lowered. Living in Central Russia 5-6 year-old children with scoliosis of I-II degree are noted to have typical signs of evident inhibition of physical development. It lies in lowering of somatometric indices, decrease of body muscles’ strength and level of their tolerance, degree lowering of spinal column mobility.

Yu. Vatnikov, Zavalishina S.Yu, Seleznev S.B, Kulikov E.V, Notina E.A, Rystsova E.O, Petrov A.K, Kochneva M.V, Glagoleva T.I Online First: October 03, 2018Orderly muscle activity in elimination of erythrocytes microrheological abnormalities in rats with experimentally developed obesity
Yu. Vatnikov, Zavalishina S.Yu, Seleznev S.B, Kulikov E.V, Notina E.A, Rystsova E.O, Petrov A.K, Kochneva M.V, Glagoleva T.IBackground: Difficulties of the earliest stages' detection of erythrocytes' microrheological abnormalities' development at obesity are connected with falling out of clinicians' field of view of persons with first signs of this pathology. It dictates the necessity of experimental investigations' fulfillment on laboratory animals with just developed obesity in them. Earlier in this pathology, there was shown high efficiency of non-medication impacts in relation lowering of the value of body mass and weakening of thrombocyte and vascular dysfunctions. At the same time, there is still no clarity in the question about the impact of regular exercise on erythrocytes' microrheological features at the beginning of obesity development.
Methods: 91 of health male-rats of Vistar line at the age of 2.5-3 months , into the investigation. 29 animals of them had experienced no impacts and composed the control group. 62 rats had obesity developed by prescribing them cardioangionefopathogenic semisynthetic diet. Then these rats were casually divided into experimental (32 rats) group and control group (30 rats). Rats from the experimental group during 60 subsequent days experienced daily exercise on a horizontal treadmill. There were used biochemical, hematological and statistical methods of investigation.
Result: As the result of obesity development the rats turned out to have steady developing increase in systolic and diastolic pressure. At regular exercise, on the treadmill, the rats were noted to have a gradual decrease of their values during 60 days of investigation to the level of the norm. During obesity development lipids' peroxidation activated in rats' erythrocytes because of activity weakening of their antioxidant protection. On the background of muscle activity in rats with obesity the content of lipids' peroxidation products in erythrocytes progressively decreased and by the 60th day of experiment reached the control level of healthy rats. At obesity development in rats, there was found a reliable decrease of erythrocytes-discocytes quantity in blood. It was accompanied by the increase of reversibly and irreversibly changed erythrocytes' quantity in examined animals' blood. Their values were returning to control the level of healthy rats during 60 days of regular muscle activity. At obesity development in rats, there was found the quick rise of erythrocytes' sum in aggregate and these aggregates' quantity at lowering of free erythrocytes' number. Their quantity returned to control values to the end of 60 days of exercise. During experimental obesity modeling, we noticed very early in rats' blood decrease of erythrocytes-discocytes' quantity, the level rise of their reversibly and irreversibly varieties with a strengthening of their aggregative ability. It takes place in the background of the weakening of erythrocytes' antioxidant protection and activation of lipids' peroxidation in them. Regular lasting muscle activity can eliminate existing erythrocytes' microrheological features' abnormalities in rats with recently developed obesity.

N.N. Ratini, N. L.P. Trisnawati, G.N. Sutapa Online First: October 03, 2018The Quantity and Leukocytes Components as Biological Dosimeters on the Radiation Workers at Radiology Installation RSUP Sanglah Hospital Denpasar
N.N. Ratini, N. L.P. Trisnawati, G.N. SutapaAbstract: Regularly, the radiation monitoring program is applied to all the radiation workers using physical dosimeter, in the TLD form, badge films and others. The monitoring from the biological aspect is called biological dosimeter as an indicator of the organ damages that caused radiation exposure, especially on the radiation workers in the radiology installation RSUP Sanglah Hospital are not much studied.
Objective: This study aims are determining of the quantity and leukocytes components differences that caused environmental radiation exposure in direct contact radiation and direct non-contact radiation workers.
Data and method: The research data was obtained through the blood test at RSUP Sanglah Hospital Denpasar with diffraction count method. The extent of the leukocyte component is determined by the absolute count method. The population in this study were 70 people with the total sample were 35 direct radiation radiation workers (30 radiograper people, 5 medical physicists) and 35 direct non-contact radiation workers (12 nurses, 18 administrative personnel, 5 medical records) .
Results: Generally, the blood cell quantity values (hemoglobin-HGB, RBC-erythrocyte, leukocyte-WBC), as well as the percentage of WBC components (basifil-Baso, eosinophil-Eos, neotrifil-Neo, lymphocytes -Limfo and Monocyte-Mono) for radiation workers and non-radiation workers are in the normal range. However, in the normal range, radiation workers have a wider range than non-radiation workers, indicating that radiation workers have undergone a stochastic radiation effect. While the quantity of blood cells over the age range of radiation workers and non-radiation workers alike shows the stochastic effect tends to appear in the productive age of 31-40 years old. For the age of 21-30 years is still normal caused by a relatively new work mass. But at the age of 41-50 tends to approximate the deterministic effect, this can be understood because the mass of work that has been long enough.
Conclusions: For long time in the radiation field area which increase the deterministic effect the cells of the human body response. Primarily, biological indicators of radiation as a dosimeter are used.

Andhika Yudistira Online First: October 03, 2018Effect of mesenchymal stem cells on Mycobacterium tuberculosis growth: in vitro study
Andhika YudistiraBackground: TB infection remains one of the major health problems in Indonesia. While stem cells have been developed as new hope and challenge on medical aspect in treatments of various disease. Mesenchymal stem cells applications in treating tuberculous infections remain controversial.
Objectives: To evaluate interactions between mesenchymal stem cells (MSCs) and Mycobacterium tuberculosis (MTB) in one co-culture growth.
Methods: This study is an experimental in vitro study, where MTB H37Rv strain and BMSCs were co-cultured in the single medium. MTB with 0.5 McFarland and 5000 cells/cm2 of MSCs were co-cultured in RPMI medium, MTB 0.5 McFarland in RPMI and 5000 cells/cm2 MSCs in RPMI are being our control groups. Acid-fast staining bacilli (AFB), PCR, TB culture were evaluated between the 3 groups on day 3, day 7, and day 9 of observation.
Result: AFB, PCR, and TB culture were found positive (+) in both TB control group and also the co-culture group (+) on day 3, day 7, and day 9. While the AFB, PCR, and TB culture in MSCs control group were found negative (-) on day 3, day 7, and day 9. Cyclus threshold PCR values between co-culture group and control groups were not significantly different (p>0.05). Viable MSCs are hardly found in co-culture group compared with MSCs control group. The difference was significant between co-culture group and MSCs control group. (p<0.05).
Conclusion: In this co-culture study, MSCs do not affect MTB growth. Instead, there were suppression of Mesenchymal Stem Cells growth in this co-culture study.

Selfi Handayani, Ari Handono Ramelan, Kristanto Yuli Yarsa, Dono Indarto, Bambang Purwanto, Koosnadi Saputro, Didik G Tamtomo Online First: October 03, 2018Laser acupuncture on ST36 promotes serum protein concentration in adolescent rats
Selfi Handayani, Ari Handono Ramelan, Kristanto Yuli Yarsa, Dono Indarto, Bambang Purwanto, Koosnadi Saputro, Didik G TamtomoBackground: In the last decade, laser technology has been used in medicine for diagnostic and therapeutic purposes. The low-level laser has been applied to acupoints (laser acupuncture/LA) which had potential to stimulate the expression of enzyme and hormone in various biological processes. Since the enzyme and majority of hormone is a protein or its derivatives, this study aimed to measure serum protein concentration in adolescent rats treated with laserpuncture.
Methods: Forty male three weeks old Wistar rats weighed ≥ 40 g were randomly divided into either group A or B. Each group was evenly divided into four subgroups which were a negative control and treated with LA on GV20, ST 36 or combination of GV 20+ST 36 respectively. These acupoints were then stimulated by continuous wave cold red laser (635-680nm/5mW) generated by KX Laser GX-2000B. LA was performed at 60 seconds/day for ten days (group A) and 15 days (group B). Venous blood was collected from sinus orbitalis, and Serum protein concentration was measured using the Bradford assay. The data were analyzed using ANOVA test and p-value < 0.05 was considered as significant. Results: A higher protein concentration was observed in ST36A group (76.326±15.084 mg/ml) and GV20+ST36A group (75.044±15.346 mg/ml) compared with control group A (67.338±12,476 mg/ml).
Meanwhile,GV20A group had lower protein level than control group at 57.838±11.042 mg/ml. By contrast, all treated rats in group B had higher protein concentrations (74.158±12.754; 79.373±7.265 and 77.854±2.552 mg/ml respectively) compared to control group B (72.292±7.987 mg/ml). However, the difference in protein concentrations in both groups did not statistically significantly (p=0.161 and p=0.541 respectively). Conclusion: LA on the ST 36 acupoint has the greatest effect on increasing protein concentration in male adolescent Wistar rats although the difference was not statistically significant.

A Comparative Study of the Neuro-Ophtha (NO) Lollipop and Hertel Exophthalmometers in Measuring Anterior-Posterior Globe Position among Patients without Orbital Disease
Amanda Juwita Sendjaja, Buenjim Mariano, Hernando L Cruz Jr Online First: October 03, 2018A Comparative Study of the Neuro-Ophtha (NO) Lollipop and Hertel Exophthalmometers in Measuring Anterior-Posterior Globe Position among Patients without Orbital Disease
Amanda Juwita Sendjaja, Buenjim Mariano, Hernando L Cruz JrObjective: To compare the Neuro-Ophtha (NO) Lollipop against the Hertel exophthalmometer, which is as gold standard, in measuring anterior-posterior (A-P) globe position among patients without orbital disease.
Methods: 423 patients without history of orbital problems, aged 18 years old and above were recruited. Measurements of A-P position of globe were done using Hertel (Ophthalmologist A) and NO Lollipop (Ophthalmologist B) independently on the same patient, in two separate rooms and the same day.
Results: Results showed the A-P globe position measurements were significantly higher using the NO Lollipop as compared to Hertel exophthalmometer. After log transformation of data, a mean difference in the A-P globe position of 0.04977 (log10) or 1.12mm (95% CI 0.12mm to 2.12mm) between Hertel and NO Lollipop readings on the right eye (OD) and 0.04652 (log10) or 1.11mm (95% CI 0.11mm to 2.11mm) on the left eye (OS) were obtained. Significant difference in the mean A-P position readings between Hertel and NO Lollipop exophthalmometer were seen in this study on OD (t=25.2, p=0.000) and OS (t=22.5, p=0.00).
Conclusion: In patients without orbital disease, NO Lollipop overestimates the anterior-posterior globe position by as much as 1.11-1.12 mm compared to Hertel exophthalmometer. In clinical setting, NO Lollipop may be used as an alternative instrument when Hertel exophthalmometer is not available.

Associations between nutrition attitudes, socioeconomic status, and vitamin E intake on blood pressure among elderly with hypertension in Klaten, Central Java-Indonesia
Triagung Yuliyana, Kusnandar Kusnandar, Diffa Hanim Online First: October 03, 2018Associations between nutrition attitudes, socioeconomic status, and vitamin E intake on blood pressure among elderly with hypertension in Klaten, Central Java-Indonesia
Triagung Yuliyana, Kusnandar Kusnandar, Diffa HanimBackground: Hypertension is a health problem which affects many elderly patients. Indirectly, the attitude of elderly patients influences food selection. With a better attitude, elderly individuals will have a better understanding of the type and amount of food to be consumed. Good nutrition attitudes can reduce or prevent malnutrition, control hypertension and reduce cardiovascular complications. The mechanism of hypertension has been associated with oxidative stress, which is related with the intake of high antioxidant foods, such as vitamin E. Antioxidants, when consumed in an adequate amount, can help the adaptation or adjustment to the changes that occur in the body. Antioxidants also increase body cells turnover that has the potential to extend the life of elderly patients. Method: This study is a descriptive, observational, cross-sectional research. The study was conducted in May 2017. The sample size was 125 elderly patients with hypertension, taken with purposive random sampling method. Vitamin E intake was measured with 2x24 hour Food Recall and Qualitative Food Frequency Questionnaires (FFQ). Other data were collected through a questionnaire, then analyzed using multiple logistic regression. Results: Correlation test showed chi-square test results that there was a correlation between nutritional attitude (p = 0,026, OR = 6,650, CI 95% = 1,319-33,517) and vitamin E intake (p = 0.043, OR = 2,147, CI 95% = 1,015-4,542) with blood pressure in elderly hypertension patients, but not on socioeconomic status (p = 0.230, OR = 0,583, CI 95% = 0,241-1,141). Conclusions: The multivariate analysis showed that nutritional attitude was dominant factors with statistically significant p = 0,023, OR = 1,50, CI 95% = 2,14-17,21.

Impact of daily wearing of medioprophylactic clothes on the state of locomotor system of 5-6-year old children with scoliosis of I-II degree
AA Bikbulatova, EG Andreeva Online First: October 03, 2018Impact of daily wearing of medioprophylactic clothes on the state of locomotor system of 5-6-year old children with scoliosis of I-II degree
AA Bikbulatova, EG AndreevaBackground: Efficient variants of scoliosis correction should be applied maximally early, as soon as its first symptoms are detected. One of such variants is daily wearing of medioprophylactic clothes (MPC). At the same time, dynamics of morphological and functional characteristics of locomotor apparatus in 5-6-year old children with scoliosis against the background of their daily wearing of MPC is still studied rather poorly.
Method: Dynamic indices of locomotor apparatus of 5-6-year old children with scoliosis of I-II degree who wore author-designed MPC daily for 6 months were tracked.
Results: Examination of 5-6-year old children with scoliosis of I-II degree was noted to have typical manifestations of physical development inhibition in their initial state such as lowered somatometric indices, decreased body muscle strength, lowered tolerance level, and reduced spinal mobility. Conclusion. Daily MPC wearing for half a year provided children with scoliosis evidence of lower spinal curvature, an increase in body muscle strength, and a closer tolerance of relevant indices to control values.

Richa Syafni Putri Wulandari, Harry A Asroel, Askaroellah Aboet, Fotarisman Zaluchu, Sutoyo EliandyChronis suppurative otitis media (CSOM) with cholesteatoma may disturb the balance between bone formation and resorption. The destruction caused by cholesteatoma growth can reach the surrounding areas due to dermal enzyme and acid production. Matrix metalloproteinase-2 (MMP-2) is a zinc proteolytic enzyme and calcium endopeptidase that degrades extracellular matrix. This cross-sectional analytical study aimed to determine the association between MMP-2 expressions and bone destruction degrees due to cholesteatoma in patients with atticoantral CSOM at Haji Adam Malik Central General Hospital, Medan Indonesia. Cholesteatoma was taken from 40 cases of CSOM and then made into paraffin blocks. MMP-2 overexpression was more dominant in paraffin blocks taken from patients with moderate degree bone destruction (47.5%). This study concluded that MMP-2 was significantly associated with the degree of bone destruction (p=0.002).

Chieng Jin Yu, Abdul Hanif Khan Yusof Khan, Lee Bang Rom Online First: December 01, 2018A case report of eosinophilic colitis in an elder
Chieng Jin Yu, Abdul Hanif Khan Yusof Khan, Lee Bang RomPrimary eosinophilic gastrointestinal disease (EGID) is a spectrum of disease characterized by eosinophilic infiltration along gastrointestinal tract without other causes of eosinophilia, with Eosinophilic colitis (EC) as the rarest form.
Case:
We reported a rare case of Eosinophilic colitis, where a 69-year-old gentleman presented to us with chronic diarrhea associated with mild abdominal pain for 6 months. He had significant eosinophilia 9.0%, with the colonic biopsy reported as mildly distorted glands with markedly increased eosinophils (>40/high-power field) in the lamina propria. Otherwise, his allergic skin testing was normal. His diarrhea was resolved with a course of steroid, with repeated blood test showing improving eosinophilia of 4.5%.
Conclusion:
EC is rarely reported. However, this could be underdiagnosed. All chronic diarrhea cases should be offered colonoscopy with biopsy to rule out Eosinophilic Colitis.
